What makes a funding evidence claim high risk

Three signal types drive the majority of PTAC challenges. Each has a distinct remediation pathway.

Absent Evidence

No direct, prospective evidence exists for the funded population. The submission relies on extrapolation from adjacent populations, surrogate outcomes, or non-randomised data — PTAC's most common basis for a Not Recommended or Deferred decision.

Indirectness Mismatch

Trial endpoints do not map to the clinical outcomes PHARMAC needs to fund — e.g., PFS instead of OS, or biomarker response instead of quality of life. High risk when the submission asks PTAC to accept a surrogate as clinically meaningful.

Equity Evidence Gap

Evidence specifically addressing outcomes for Māori, Pacific, or high-deprivation populations is absent. Under PHARMAC's Te Tiriti obligations, this gap can block funded access or generate binding conditions — even when the overall efficacy evidence is acceptable.
